Yes. Aslan Health can provide an itemized receipt showing the services you received and the amount you paid.

What Should I Bring to My Appointment?
Please bring:
- A photo ID, if available

- A method of payment, including an HSA or FSA card if you plan to use one
If you do not know the name or dosage of a medication, bring the medication bottle or a photo of its label if possible.
What If I Cannot Pay the Full $75?
We do not want financial hardship to prevent someone from receiving needed medical care.
If you cannot pay the full cost at the time of your visit, payment plans are available. Aslan Health also offers a sliding-fee scale based on household income and family size. An application and proof of income are required.
