Aslan Health Joins the Sage Plus Program: Expanding Our Commitment to Whole-Person Care

At Aslan Health, we are excited to announce that we are now a Sage Plus Clinic, partnering with the Minnesota Department of Health to provide free heart health screenings and lifestyle change services to eligible individuals. This opportunity allows us to deepen our commitment to caring for the uninsured and underinsured while further integrating our Whole Person Care approach—addressing not just physical health but also emotional and spiritual well-being.

What Is the Sage Plus Program?

Sage Plus is a state-run initiative focused on preventing heart disease and stroke by offering comprehensive screenings, health education, and lifestyle coaching to those who might not otherwise have access to these vital services. It is specifically designed for women between the ages of 35-64 who meet the income and insurance eligibility of the Sage Breast and Cervical Cancer Screening Program, ensuring that those at the highest risk receive proactive care at no cost.

Through this program, we can now provide:
Heart Health Assessments – Including screenings for cholesterol, blood sugar, and blood pressure
Lifestyle Change Support – Coaching and education on nutrition, exercise, and stress management
Barrier Reduction – Helping patients navigate challenges like transportation and financial hardship to access healthcare
Ongoing Preventive Care – Encouraging early intervention for long-term heart health
