That’s why we’re proud to be a provider for the Sage and Sage Plus Programs—initiatives from the Minnesota Department of Health designed to catch serious health conditions early, when they’re most treatable.
What is the Sage Program?
The Sage Program provides free breast and cervical cancer screenings to qualifying individuals. These screenings include clinical breast exams, Pap tests, pelvic exams, and mammograms. Early detection is critical—when found early, these cancers are much easier to treat, leading to better outcomes and often, lives saved.
If you’re a woman who is 40 or older, uninsured or underinsured, and meet income guidelines, you may qualify for Sage. At Aslan Health, we walk with you every step of the way—from screenings to referrals for follow-up care if needed.
What is Sage Plus?
Sage Plus builds on the foundation of cancer screening by addressing heart health and lifestyle change support. This program focuses on reducing the risk of chronic conditions like high blood pressure, high cholesterol, and diabetes—all of which disproportionately affect low-income and underserved populations.
Through Sage Plus at Aslan Health, you can receive:
- Blood pressure and cholesterol screenings
- Diabetes risk assessments
- Health education and personalized wellness support
- Lifestyle counseling to support long-term health goals
These services are provided free of charge to those who qualify.
