Prostate cancer is the second most common cancer in men worldwide and a leading cause of cancer-related deaths. About 1 in 8 men will be diagnosed during their lifetime, and while many cases progress slowly, early detection can make all the difference.
Why Early Detection Matters
Prostate cancer often doesn’t show symptoms in its early stages. That’s why routine screening—such as the PSA (prostate-specific antigen) blood test and, in some cases, a digital rectal exam (DRE)—is so important. These tools help identify cancer before it spreads, offering men the best chance at effective treatment and long-term health.
American Cancer Society Screening Guidelines
The American Cancer Society (ACS) recommends that men talk with their healthcare provider about the pros and cons of screening, especially those at higher risk. Key recommendations include:
- Start the conversation at age 50 for men at average risk.
- Begin at age 45 for men at higher risk (including African American men or those with a first-degree relative diagnosed before age 65).
- Begin at age 40 for men with multiple family members diagnosed early.
PSA Testing Frequency
- If PSA is 2.5 ng/mL or higher: Consider annual screening.
- If PSA is under 2.5 ng/mL: Testing every two years may be appropriate.
Who Should Not Be Screened
Men with less than a 10-year life expectancy generally do not benefit from prostate cancer screening and may avoid unnecessary testing or procedures.
Personalized, Informed Decision-Making
There’s no one-size-fits-all answer when it comes to prostate cancer screening. Each man’s health status, family history, and personal values matter. The ACS and other major medical organizations emphasize the importance of shared decision-making—a conversation between patient and provider to make the best individualized choice.
